Travis Foutz, 44, of Farmersburg, Indiana passed away on Monday, October 19, 2015 at Terre Haute Regional Hospital.
He was born on October 11, 1971 in Sullivan, Indiana the son of Barry Foutz and Janet Whitman Soules. Travis was a member of the Terre Haute Shrine Club.
Survivors include his father Barry (Vondelius) Foutz, mother Janet (Randy) Soules and step-brother Jason Foutz.
A memorial service will take place at a later date. Arrangements were entrusted to Brust Funeral Home.
